Cornerstone Children's Network: Transition Services
- Individualized transition planning for students ages 14-21
- Community based activities and assessments
- Daily activities to include shopping, leisure, volunteer and work experiences
- Full and part time opportunities
- Vocational development to include job exploration, job shadowing, interviewing skills, resumé and portfolio development
- Activities of daily living to include self care skills, household chores, cooking, shopping, etc.
- Formal vocational evaluations and interest inventories completed by trained staff and provided to local school districts and the Office of Rehabilitative Services (ORS)
- Assistance given to families with the transition process
